If you obtained healthcare coverage under Obamacare you need to Form 1095-A on your 2016 tax return. The form will be mailed and available also through the HealthCare.gov account. Form 1095-A will report the total monthly health insurance premiums paid to the insurance company you selected through the Marketplace. It also details the advance payments to the premium tax credit that were paid directly to your insurance company.
If you do not receive Form 1095-A or the information is incorrect you must contact your Marketplace directly. This form comes from a third party the IRS is not going to be able to answer questions about this form.
It is necessary for you to have Form 1095A and bring it with you when you file if you had the ACA health insurance for any portion of the year.
